Summary:
The Case Manager assumes accountability and responsibility for managing the hospitalization of a caseload of patients from pre-admission through one-week post-discharge. Facilitates coordination of care by all members of the health care team to facilitate achievement of appropriate outcomes. 
Responsibilities:
1.Evaluates and assesses patients' homegoing needs and provides case management to meet these needs for safe transition/discharge.
2.Assists in the planning and implementation of medical care and coordination of services with other patient care areas, both within and outside of the organization and links families with community resources/services.
3.Assesses and procures community resources for families, while assessing for changing patient needs throughout the course of care with appropriate referrals for consultation, as needed.
4.Assists with development and implementation of materials necessary to facilitate patient care, patient/family/caregiver education and staff education related to discharge/transition planning.
5.Demonstrates leadership and interpersonal relationship skills which enhance communication and promote conflict resolution. Communicates effectively with patients/families, the interdisciplinary team and outside resources.
6.Demonstrates the knowledge and skills necessary to provide care for the physical, psych/social, educational, and safety needs of the patients served regardless of age.
7.Other duties as required.
 
Other information:
Technical Expertise
1.Experience in case management is preferred.
2.Experience in healthcare is required.
3.Must possess excellent oral and written skills.
4.Ability to move freely about the hospital and read/interpret written and graphical data is required.
5.Proficiency in MS Office [Outlook, Excel, Word] or similar software is required. Epic software or similar EMR software is preferred.
Education and Experience
1.Graduate from an accredited school of nursing, licensed and currently registered as a professional nurse in the State of Ohio. Bachelor's degree in Nursing [BSN] is required Master's degree is preferred.
2.CCM or CCCTM certification preferred.
3.Current Health Care Provider BLS training from the American Heart Association is required.
4.Deals with confidential information and/or issues using discretion and judgment.
5.Minimum years of relevant experience: 3 to 5 years is required.
6.Years of experience supervising: None
