Position Summary: We have an exciting opportunity to join our team as a Sr Coordinator, Quality Improvement.
Assists the Director of QI/Clinical Systems Integration, toward the implementation of the Quality Management Plan. With leadership collaboration, facilitates improvement initiatives by investigating workflows and analyzing data with the deliberate intent of developing recommendations for improvement in order to integrate and maintain PCMH standards and guidelines. Also collaborates with leadership/staff managing regulatory compliance, patient satisfaction and or complaints as well as others who impact on the overall quality improvement vision of the organization. This position/role is responsible to supervise and direct the QI coordinator(s) activities as needed.
Responsible for the supervision of the QI coordinator as it pertains to PDSAs and/or Medical Home standards
Participates in the planning, development, presentation and delivery of education programs in support of NCQA PCMH standards
Conduct on-site visits to assess and assist care teams through changes to workplace culture, efficiency, documentation and use of technology needed to adapt to continuous changes in healthcare standards and regulations
Facilitate development of training curricula relevant to PCMH and educate providers on clinical information technology, process design, change management, and evidence based practice to empower and enable improvements in quality and patient care experience
Participates in assigned standing committees and improvement teams. Acts as Team Leader or Facilitator, for improvement teams
Works with practices to understand quality results and develop plans to improve quality of care
Promote workflow redesign and structured data entry to improve care coordination, data reporting and analysis
Collaborates with the Information Technology team to streamline and use technology, ie automation to facilitate the overall quality improvement activities, which would include data collection, metrics development, publication of metrics and or improvement results
Collaborates with the appropriate clinical and administrative leadership in developing/revising policies and procedures
Assists in the integration of PCMH standards and guidelines into the overall Quality Improvement Plan
Facilitates improvement initiatives, which includes but not limited to, monitoring trends in metrics, works collaboratively with physicians, administrative leadership, professional and paraprofessional staff in rapid cycle PDSAs
Minimum four years of experience as a quality improvement coordinator, specialist, or similar role in a healthcare setting
Experience with State and Federal compliance regulations and standards
Skilled in MS Office
PCMH Certified Content Expert
Certified Professional in Healthcare Quality (CPHQ) certification
Experience with deliverables and management for an FQHC and Article 28
