The Quality and Clinical Transformation Director will support programs to improve patient experience and relations as it relates to Behavioral Health nursing quality. Oversees clinical support functions including infection control, laboratory services, patient rights, quality, and safety programs. Collaborates with Mount View Nursing Home Nursing and Quality functions to assure consistent level of standards are maintained and efficient use of resources are utilized.

Essential Duties and Responsibilities:
In conjunction with leaders, is responsible for supporting the review of population health management initiatives, continuity of care, and the evaluation of clinical effectiveness for improved outcomes.
Serves as the voice of the patient by creating and implementing plans for improving the patient experience; collaborates with leaders and directs patient satisfaction survey processes; provides consultation to leadership and staff to improve patient experience; and administers a framework for continuously monitoring and improving the quality of care and service provided to our patients, residents and clients.
Reports directly to the Board on Quality and Compliance issues with primary responsibility for an effective Corporate Compliance program.
Assists Executive Management team members in resolving operation issues in regards to patient care including patient grievance reporting, intake, investigation and disposition. (compliance)
Provides guidance for organizational process improvement and process redesign efforts in the dimensions of clinical quality, service delivery and patient experience.
Provides support in the interpretation, compliance and maintenance of practice standards to match external requirements of all regulating and accrediting bodies including supporting the Human Services Operations Executive in managing and maintaining Joint Commission Accreditation.
In partnership with the CEO, ensures compliance with the Medical Staff Bylaws, credentialing and regulatory agencies; assists in developing and maintaining policy and procedure manuals for the Medical Staff; collaborates with physician leadership to ensure the Medical Staff standards are met; and assists with orientation programs for the Medical Staff. Chairs the Nurse Practice Council. Assumes responsibility in overseeing auditing and evaluating nursing practice. Presents nursing and quality concerns to the Executive Management team, Board and Medical Staff when appropriate.
Ensures the systematic collection of data regarding compliance and quality initiatives and the provision of forums for routinely reviewing this information with multi-disciplinary teams. HSO
Directs and monitors Process Improvement Team and works with Talent Development to develop related training.
Formulates and recommends goals and objectives as part of the annual Quality Improvement Plan for performance improvement and Continuous Quality Improvement (CQI) for NCHC.
Prepares an annual Compliance and Quality Improvement report to the Board that outlines outcomes delivered through NCHC clinical programs and emphasizes the value proposition of NCHC.
Minimum Education • Bachelor's degree in nursing • Current Wisconsin Registered Nurse license • Graduate of an accredited nursing degree program
Minimum Work Experience Five to ten years' of progressive hospital management experience
Required Skills • Current Professional Nurse Registration from the Wisconsin State Board of Nursing • Current BLS (CPR) Certification • Effective oral and written communication skills
Driving Requirements: • Valid Wisconsin driver's license and appropriate auto insurance coverage as required by North Central Health Care. • Must have the use of a personal vehicle.
Preferred: • Baccalaureate degree in Nursing preferred
